What is Ambulatory Blood Pressure Monitoring (ABPM)

An ABPM will measure your blood pressure over a period of time. This will typically be 24 hours. Your blood pressure is monitored outside the clinic as you follow your normal daily routine. It is well documented that blood pressures measured in a clinic are not always representative of everyday blood pressure readings

Price: £150

Why would you need ABPM?

Ambulatory blood pressure monitoring may be recommended for several reasons, including

  • Have high blood pressure based on readings at your provider’s office, but you haven’t yet started treatment.
  • Need changes to your blood pressure medications.
  • Still have high blood pressure despite medications.
  • Are taking other medications that may affect your blood pressure. · Have fainting episodes or hypotension (low blood pressure).
  • Have high blood pressure during pregnancy.
  • To evaluate how well medication is controlling your blood pressure
  • To diagnose long-term high blood pressure
  • To assess when your blood pressure goes up or down
  • To exclude higher-than-normal readings caused by ‘white coat syndrome’

What are the advantages of ABPM?

When your blood pressure is measured, the reading will record the level at the time of measurement. But blood pressure varies throughout the day, so to get an accurate idea of your overall health, ABPM takes many readings over an extended period, usually up to 24 hours.

Another problem with single-point measurement is that some patients get anxious when they visit a clinical environment, leading to temporarily raised levels known as ‘white coat syndrome’. ABPM avoids this by recording your blood pressure while you’re going about your everyday life.

What happens during the fitting of the ABPM?

  • The technician will explain what will happen during the monitoring period
  • You will be asked if you consent to having the ABPM fitted
  • It may be necessary for you to remove some of your upper body clothing during the fitting of the ABPM
  • The fitting of the ABPM generally takes about 15 minutes. Your appointment should not take longer than 30 minutes
  • A small pouch containing an ABPM will be fitted to your waistline using a waistband or shoulder strap
  • The ABPM will inflate a blood pressure cuff, this is normally fitted to your non-dominant arm
  • The ABPM monitors and records all the data provided by the cuff during the monitoring period

How do you prepare for ambulatory blood pressure monitoring?

Your healthcare provider will tell you how to prepare. Some general tips include:

  • Choose a 24-hour period that best represents your normal routine. This may mean choosing a weekday rather than a weekend.
  • Plan to keep a journal of what you do during the 24-hour period. This includes when you wake up and go to bed, when you take any medications and when you eat. You should also record any symptoms you experience (like dizziness or shortness of breath).

Preparation also involves knowing in advance what you need to avoid. During your 24-hour period, you shouldn’t:

  • Engage in strenuous physical activity.
  • Shower or swim.
  • Remove any part of the device without first asking your provider.

What to Expect

Comfort

The device is designed to be lightweight and non-intrusive, although some patients may experience mild discomfort during cuff inflation

Limitations

Patients should avoid getting the device wet (no showers or baths) and may need to limit certain activities during the monitoring period

Results

After the monitoring period, the data is analyzed, and a report is generated, typically available within a few days

What your readings mean

  • If your blood pressure is below 140/90 mm Hg at the surgery, it will be considered normal.
  • If your blood pressure is above 140/90 mm Hg at the surgery but your average ambulatory or home reading is below 135/85 mm Hg, it will be considered normal.
  • If your blood pressure at the surgery is 140/90 mm Hg or above and your average ambulatory or home reading is 135/85 mm Hg or higher, it will be considered high.
