What is an ambulatory ECG?

An ambulatory ECG is a small, portable heart monitor that records your heart’s electrical activity over an extended period, often 24 hours or more. It’s used to detect irregular heart rhythms (arrhythmias) that might not show up during a standard ECG at the doctors.

Because it monitors your heart over time as you go about your daily routine, it gives a more accurate picture of how your heart behaves under real-world conditions.

How is it different from a standard ECG?

A traditional ECG, sometimes called a resting ECG, records just a few seconds of heart activity while you’re lying down. That’s useful, but it can miss intermittent issues, especially if they come and go unpredictably.

An ambulatory ECG records continuously for 24, 48, or even 7 to 14 days. That’s especially important if you’re experiencing symptoms like palpitations, dizziness, or fainting spells that don’t happen every day. By capturing a longer window of activity, it can detect irregularities a standard ECG might miss.

Who might need one?

Your doctor might recommend an ambulatory ECG if you’re experiencing:

What can ambulatory ECG detect?

  • pericarditis (inflammation of the tissue around the heart)
  • cardiomyopathy (swollen or thickened heart walls)

As the monitor records around the clock, it can capture infrequent heart rhythms while you’re working, relaxing, exercising or sleeping.

What does the test involve?

If you’re referred for an ambulatory ECG, here’s what typically happens:

  • A small device is placed on your chest using adhesive pads.
  • You’ll wear the device continuously for a set period, from 24 hours to 7 days or more.
  • You’ll be asked to go about your normal daily routine.
  • Some devices allow you to note symptoms using a button or paired mobile app.
  • After the test, the data is analysed using software and interpreted by a clinician.

You can sleep with it on, and some models are water resistant, meaning you can shower while wearing it, though long soaks or swimming might not be advised.
